1. You do not agree with the results of a school-based evaluation.
2. The parents/guardian’s input and opinions are being disregarded. 
3. The school district does not have the appropriate personnel to conduct a specific    evaluation.
4. The school district’s evaluation is missing some vital information.
5. The school district wants to discontinue a service or program that you feel is still relevant.
6. Parents/guardians suspect another co-morbid condition has not been identified.
7. Your child is not making progress and is regressing.
